I think I need some help or advice

Hi Guys! Hope everyone had a good weekend.
I think I need some help or advice…
I can only see my specialist once every 4-6 months. My next appointment is on Tuesday and I don’t know what to ask him! I have done everything he’s said… been wearing the bite guard he made (it’s not really a splint – not testing went into in before hand – just mold of my mouth and he adjusts it whenever I see him) and going for work on my muscles in my face and neck every week. For the past 5 months I’ve felt the same… no improvements. My problem arose 2.5 yrs ago out of no where – just literally woek up with it. I had clicking last yr on one side but no pain. Pain started the beginning of this year and is every day. Both of my discs are displaced but reducing. The clicking went away this year. Opening has always been good. He tells me not to worry about the discs. But since I am still uncomfortable and in pain, I don’t know what to ask him anymore. Last time I saw him he wanted me to go to the pain clinic… but I don’t want to do that (yet). Any advice? I’m not sure if I’m missing a part of the puzzle or not doing something that I should be or if I should find someone to give me another opinion (no idea who though).

Pain is around the joint, below it, and sort of under the cheek bone – along that edge. It’s like a warm aching pain. My left side doesnt pop anymore but it cracks slightly upon very wide opening on both sides – a very dull, quiet crack that I can just feel and hear. I just feel like I’ve been doing SO much PT… every week and it’s not helping anymore. I feel the same after about a day. I don’t know how to know if my discs have been recaptured either. I want to ask for another MRI since I feel different but my last (and only) one was 6 months ago.

Thanks Luis. I just had a look at your link. It’s a little confusing lol what does this mean? I see it can tell me what muscles are affected – I have someone work on my muscles weekly. hmmm

it shows the muscles that cause pain on a diferente área of the body. if you touch TP ( black crosses) you might feel pain in the red areas

go for head cheeck área and you see the muscles that can Project pain in that area

It means that you might have pain in the tmj área that comes from neck/shoulder, so if you try to deal with the pain in the tmj área it´s not going to work

Do you have these pains with the guard/mold on or both: with and without? Do you feel that you are biting too strong on it?
I am not sure what you actually mean by the mold of your mouth? Is it thick?

This is what it looks like. I’ve noticed no change with or without it. I’ve gone through periods of time using it and not, no difference. I don’t think it’s very thick but I do think I bite down on it more just because there’s something foreign in my mouth.

if your bite is not correct, it will cause you to bite more, straining your joint as well as your muscle, maybe you can ask your doctor why you are still in pain

“I do think I bite down on it more…” — IMO, that’s because it is missing the anterior contacts and possibly the canine guidance. Thus your chewing muscles got very tired and ache. IMO, the improvement with the clicking and popping might show that you do need to raise the VDO but you should not be clenching or biting hard on anything throughout the day (nor at night). If you can get a new MRI (and the insurance pays) then IMO, there is no harm in doing so. Can your dentist read it well?
I hope dentists will comment on this.

No it’s just for at night. I’ll ask more about it tomorrow. I’ve had it since last fall I believe. It makes my teeth feel really weird in the morning but the feeling goes away after 15 mins. I;ve been told by 3 dentists that my bite is fine? So I have no idea. I’ve never had any changes to my bite. I’ll see if I can get another MRI.. I’m in Canada so I don’t have to pay for that (everything else though lol). I think that because they are expensive they don’t issue them often. It also took me MONTHS to get the actual MRI after ordering it. ugh. If my bite has never changed and my issue arose 2.5 years ago out of no where, no trauma or dental work, then how could my bite all of a sudden be off ? I have such a hard time understanding this 🙁
I also can’t seem to find anyone whose actually been helped from a splint… out of all of these gorups that I’m in it seems like everyone is the same. I feel like I want another opinion but no idea who to go to… I’m in a major city and cant believe how hard it is to find someone.

Tell him you are biting too heavily on the splint all day long. Ask if anterior contacts would help ease this up? Can you breath through your nose? During the day and at night?

I don’t wear it during the day. Yep, nose breather! I can ask.

Can the bite be off even if it feels fine and 3 people have checked it? It’s never felt any different or off.

yes, bite might feel fine, but your muscle and joint are taking all the pressure, it is just like if you are a parent, u will do everything for ur children, they will feel fine, but u carry all the burden, something like that

bite is check by doing a bite analysis, where we can see how ur teeth relate to your joint, and how it affects the muscles
