Would be interested to know the groups opinion of tmjd treatment which involves development of the maxilla to widen a collapsed top arch, provide appropriate space for the tongue and improve nasal breathing using an alf style device without splints. Thoughts?

Effectivement vous avez compris l’origine du problème. Un problème fonctionnel dans la petite enfance (respiration buccale. Positionnement de la langue…) et une adaptation du seul élément d’ajustement (l’articulation) dépassée par le stress…
J’essaye de soigner beaucoup d’enfants entre 4 et 6 ans avec succès en faisant de l’expansion palatine.
Par contre sur mes patients adultes et en particulier ceux ayant reçus un traitement d’orthopédie trop tardif je ne peux travailler qu’en équipe avec des ostéopathes des kinésithérapeute des médecins orl…

If I understand Patrick is saying that he works with children with success but with adults who have had ?? I think he means orthodontics too late, he can only suggest osteopaths and physical therapy.

If proper nasal airway breathing can be restored in adults along with correct tongue position by widening of the upper arch to bring the upper and lower jaws into a better functional relationship will that not improve an adults situation? Eva Willer-andersen what do you think?

HI Gaz yes theoretically expansion should help depending on your start point and need . The difficulty is finding a really good way to expand the maxilla in adults and a good dentist I found this guide quite useful to asses arch size

Thanks Eva, I know that most good practitioners favour slow expansion using an alf but in the end maxillary expansion is possible in adults. My research is also telling me that displaced discs should try and be recaptured and addressed first as otherwise many symptoms may remain. It’s certainly an interesting puzzle, involving breathing, disc position and arch development!

L’expansion palatine chez l ‘adulte ne peut se faire par les sutures que chirurgicalement. L’expansion se fait sinon par la version des dents même de manière progressive et douce.

I think that’s the aim of most TMJ dentists. I have had what you have mentioned yet I still need a ENT Surgeon to do the turbinate, tongue and tonsils to give me better airway breathing
